Kindly give us at least 24-hour notice to make changes to your appointment (210) 495-2367.
You will receive a text 4 days prior to your appointment prompted you to confirm your appointment. Appointment confirmation is required to keep your appointment.
Please arrive early for check in:
After your appointment is scheduled, you will receive an email giving you access to your patient portal. This email will be from [email protected]. Don’t forget to check your junk inbox if you do not receive it in your regular inbox. Please follow the instructions in this email to obtain access to your patient portal.
As soon as possible, please complete, or update, your medical history information. This will save you significant office time on the day of your appointment. However, if you cannot access or complete this information before your appointment, please arrive 30 minutes early so we can help you do this. If you have trouble logging in, please contact our office (210) 495-2367.
Once logged into your patient portal, you will be able to complete your medical history under the “My Health” tab.

TIP: When entering your pharmacy, click the “Add Surescripts Pharmacy” button and search for your pharmacy. You may add more than one pharmacy, but you will want to be sure you make your preferred pharmacy the “default.”

☐ Please bring a detailed list of all medications (including over the counter, vitamins, and eye drops) including dosage and frequency.
☐ Bring your primary care provider information if applicable.
☐ If you are being referred by another doctor, bring the referring provider information.
☐ Bring applicable imaging.
☐ Bring insurance cards & driver’s license (required for every visit)
☐ Bring payment for co-pays or other applicable patient share cost.
