We appreciate your trust in us for your aesthetic, facial, and oculoplastic needs. To help ensure your appointment goes smoothly, please review the following important details.

Appointment Changes & Confirmations

Please give us at least 24-hours’ notice if you need to reschedule or cancel: (210) 495-2367.

You will receive a text message 4 days prior to your appointment prompting you to confirm.

Confirmation is required to keep your appointment.

Before Your Appointment

Patient Portal & Medical History

After scheduling, you will receive an email from welcome@practicemail.com with a link to your patient portal.

Check your junk/spam folder if you don’t see it.

Once logged in, go to the “My Health” tab to complete or update your medical history.

TIP: When entering your pharmacy, click “Add Surescripts Pharmacy” to search and select your preferred location. Set your main pharmacy as “default.”

If you cannot complete your medical history in advance, please arrive early so we can assist you.

On Appointment Day

Arrival Times

• Arrive 15 minutes early if your medical history is complete in the portal.
• Arrive 30 minutes early if you have not completed your medical history online.

Seating Policy

• Due to limited space, please bring no more than one guest with you.
• Please do not arrive more than 30 minutes before your appointment.

Communication & Scheduling Updates

We make every effort to respect your time. Occasionally, it may be necessary to adjust appointments due to emergency procedures or other factors.

If changes are required, we will contact you as early as possible.

Please check and respond to voicemails or text messages from our office. These may include:

• Requests for insurance information
• Changes to appointment day or time
• Appointment confirmations

Appointment Checklist

Please bring the following with you to every appointment:

• Insurance card(s) and driver’s license
• Payment for co-pays or other patient responsibility
• Medication list – include prescriptions, over-the-counter products, vitamins, eye drops (with dosages and frequency)
• Primary care provider information (if applicable)
• Referring provider information (if you were referred by another doctor)
• Applicable imaging related to your visit
