You will receive a text message 4 days prior to your appointment prompting you to confirm.
Confirmation is required to keep your appointment.
After scheduling, you will receive an email from welcome@practicemail.com with a link to your patient portal.
Check your junk/spam folder if you don’t see it.
Once logged in, go to the “My Health” tab to complete or update your medical history.
TIP: When entering your pharmacy, click “Add Surescripts Pharmacy” to search and select your preferred location. Set your main pharmacy as “default.”
If you cannot complete your medical history in advance, please arrive early so we can assist you.
